"I Am The Maven" T-Shirt Giveaway Photo Release/Official Rules

I hereby grant I Am The Maven, Inc. (the “Company”) permission to use my likeness in a photograph or other digital reproduction in any and all of its publications, including website entries, without payment or any other consideration.

I understand and agree that these materials will become the property of the Company and will not be returned. I hereby irrevocably authorize the Company to edit, alter, copy, exhibit, publish or distribute this photo for purposes of publicizing its programs or for any other lawful purpose.  I understand and agree to have my first name, first initial of last name and hometown announced and posted at www.iamthemaven.com and/or social networking sites belonging to the Company. 

In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph. I hereby hold harmless and release and forever discharge the Company from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.

Furthermore, I understand and agree that only one entry per physical address and/or person will be accepted.  I understand that I must be registered to receive Coupons by Email to receive this giveaway (Online/Everywhere edition and/or Miami edition).  I understand that I must submit a photo in order to receive a free t-shirt.  I understand that I Am The Maven is not responsible for late, incomplete, void, corrupted, garbled, misdirected or otherwise unintelligible submissions and for any problems, bugs or malfunctions encountered with submissions.  

Promotion will run as long as supplies last.  T-shirt will be mailed via USPS in a postage-paid envelope.  Giveaway (t-shirt) may not be sold and is not redeemable for cash.  

I testify to being 18 years of age or older and am competent to contract in my own name. If the person pictured is under age 18, I certify that I am the parent or guardian of the minor child and do hereby give my consent without reservation to the foregoing on behalf of the minor child.  I have read this release before indicating my agreement with the terms by checking the "I Agree" box below.  I fully understand the contents, meaning, and impact of this release/official rules.
